Our multidisciplinary team of Registered Nurses, Physical Therapists, Occupational Therapists, Speech-Language Pathologists, and Registered Dietitians works together to help you achieve your wellness goals while coordinating care and connecting you with additional healthcare providers whenever appropriate. We can also provide personalized home-visits in the event of not being able to leave the house or for specialized care.
